Use this form to submit a request to be excused from a performance. As a reminder, all performances are required for all band members per the GCHS Band Handbook and SWCSD Curriculum guide. Should there be an
extenuating circumstance which prohibits your child from attending a performance, please fill out the form linked below.
Make up assignments are listed in Section 2 of this form as well as in the GCHS Band Handbook (section III, article F). Make-up assignments are due the Monday following the event to receive credit.

PLEASE NOTE: This system must be used 3 weeks or more in advance of an event. Should there be a last-minute emergency after the three-week window, a phone call to 614-801-6143 and email to Mrs. Laubscher must be sent as soon as possible.
If the event you are looking for is not listed on the form, the three-week window has expired and communication with Mrs. Laubscher is required via email and phone call.
